FALLS AND FALLS WITH INJURY
At Caritas Carney Hospital, a Fall Prevention Protocol is in place. The program involves an ongoing safety assessment for each patient that starts at admission and continues throughout the hospital stay. Appropriate interventions are activated per the Falls Prevention Protocol. All staff are educated about the protocol in orientation and ongoing education programs. Patients and families are provided with a written brochure describing measures for prevention and interventions needed. Additionally personal safety alarms and specialty beds are used for those patients identified at risk. Patient safety is consistently a focus of patient care.

PRESSURE ULCER PREVALENCE
At Caritas Carney Hospital a Pressure Ulcer Prevention Protocol is in place. Education to the protocol begins with orientation and is on-going including review at skills-day. Evidence-based practices including assessment for risk on admission and daily through use of the Braden Scale, as well as specific interventions for those patients identified at risk are part of our prevention protocol. Resource nurses on each unit meet with the Wound and Skincare Resource nurse to review and implement best practices. There were no hospital acquired pressure ulcers in October-November, 2007.
